Staff request submission scaffold. Staff users will be allowed to insert debit/request rows only for themselves.
Form fields planned for MVP: Period Start, Period End, Hours Worked, Hourly Rate Snapshot, Expense/Reimbursement Amount, Notes, and optional report reference. This mock form is disabled until Supabase Auth and RLS are connected.
